About Us

Southern Roots wasn’t built to be just another doctor’s office.
It was built to feel like home.

We’re bringing care back to what it used to be:

personal, familiar, and meaningful.

Here, we take time to truly get to know you and your family background, not just your chart. We’re here to listen to your concerns and find solutions while keeping in mind what is most important to you in your care, and walk alongside you, no matter where you are in your health journey.

Meet Dr. Summer Alvarez, DO

Born and raised in South Georgia, Dr. Summer Bryant Alvarez has deep roots in the area.  . After graduating from Cook High School in 2014, she earned her Bachelor of Science in Biology from Valdosta State University in 2017. She went on to attend the Philadelphia College of Osteopathic Medicine (PCOM), obtaining her medical degree as an osteopathic physician in 2021.

Dr. Alvarez completed her residency in Family Medicine at HCA Florida St. Petersburg Hospital  and became board-certified in family medicine. She has practiced in a variety of medical settings in hospitals and emergency departments across South Georgia, and serves as a Clinical Adjunct Assistant Professor in Family Medicine for PCOM. She feels teaching the next generation of physicians is a true honor and a meaningful way to give back to the profession.

Dr. Bryant Alvarez was raised in a household of medical professionals. Her mother was a registered nurse for over 20 years, and her father was a family nurse practitioner. Their example strongly influenced her decision to pursue a career in the medical field. Dr. Alvarez’s dream of opening Southern Roots DPC was inspired by her late father, Bill Bryant, a Family Nurse Practitioner, who co-founded a direct primary care office. His respect for medicine, love for his patients, and passion to help the community left a lasting impression that she hopes to carry on at Southern Roots.  She hopes to continue his legacy and make a lasting difference by providing honest and quality care to families while adding a little bit of southern charm.

Dr. Bryant Alvarez has a true passion for outpatient medicine and treasures the relationships she builds with patients and their families.

Outside of medicine, she lives with her husband, Dr. Raymond Alvarez, and their two dogs in Hahira, GA. She enjoys spending time with family, traveling, horseback riding,  SCUBA diving, hunting, deep-sea fishing, exercising, and gardening.

Why We Started Southern Roots

Today’s healthcare system often falls short, especially for residents of rural communities. Appointments are rushed, costs are unpredictable, and communication with professionals seems forced and transactional. That’s why we chose to build a different kind of practice, and set the new standards for quality care in South Georgia.

Southern Roots Direct Primary Care is our way of practicing  medicine that feels right, where the doctor knows your name, understands your history and background, and takes the time to truly care for you. At Southern Roots, you get access, transparency, and providers who are genuinely invested in helping you take care of you!

We’re proud to provide compassionate, patient-centered primary care to families across South Georgia and care for you and your family like our own.

We Saved You a Spot

We’re here when you’re ready! Whether you have any questions or want to schedule your first visit, we are happy to help you.
